We offer a full spectrum of community living and supportive living services to help individuals achieve their goals.
Community Living & Support (CLS)
Community Living and Support (CLS) services under the 1915(i) program help individuals develop the skills needed to live as independently as possible within their community. Our trained staff assist with daily living activities, personal development, and community engagement. CLS services focus on building life skills such as communication, decision-making, problem-solving, and maintaining healthy routines while promoting independence, dignity, and active participation in the community.
Supported Employment (SE)
Supported Employment services help individuals prepare for, obtain, and maintain meaningful employment in their communities. Our employment specialists work closely with individuals to identify strengths, interests, and career goals. Services may include job readiness training, resume assistance, interview preparation, job placement support, and ongoing coaching to help individuals succeed and grow in their workplace.
Individual Transition Services (ITS)
Individual Transition Services assist individuals who are transitioning into independent or community-based living. These services help individuals establish safe, stable living environments by providing support with essential household setup needs. ITS may include assistance obtaining necessary household items, basic furnishings, and other supports needed to successfully move into and maintain independent housing.
Respite Services
Respite services provide temporary relief for family members and caregivers who support individuals with ongoing care needs. Our trained staff step in to provide safe, compassionate supervision and support while caregivers take time to rest, manage personal responsibilities, or attend to other needs. This service helps maintain a healthy balance for families while ensuring individuals continue receiving quality care in a supportive environment.
